Sharing a concern

Please describe the practice, the facility involved, relevant dates, and any publicly available supporting information. Distinguish what you personally observed from information provided by others.

Do not email medical records, patient names, dates of birth, or other sensitive personal information.

The Council is an educational and advocacy organization. It does not provide medical care, investigate emergencies, or replace official reporting channels. For an immediate medical concern, contact your healthcare provider or emergency services. Formal complaints should also be directed to the appropriate facility, regulator, or oversight agency.
